The physiological sigh: one breath pattern the evidence supports

Two short breaths in through the nose, one long breath out. What the physiological sigh does to the nervous system, what the research actually found, and how to use it without overselling it.

Two short breaths in through the nose, then one long breath out. That is the whole pattern, and it happens to be one of the few breathing techniques with a proper randomised trial behind it rather than just enthusiasm. Dr Arthur Kaganovitch (Chiropractor) at Aura teaches it to patients as a genuinely useful tool for a nervous system that has been running hot for too long, though useful is a very different claim from magic, and it is worth understanding exactly what the pattern does and does not do.

What a physiological sigh actually is

The physiological sigh is a breathing pattern your body already produces on its own, most often just before you drift off to sleep or after a long cry, when an involuntary double breath seems to reset something. As a deliberate technique it has three parts: a first inhale through the nose until your lungs feel comfortably full, a second short, sharp inhale on top of it through the nose, then one long, slow exhale out through the mouth, roughly twice the length of the two inhales put together.

Mechanically, the second inhale is doing real work. Small air sacs in the lungs, called alveoli, tend to collapse a little as a normal breath runs out, and the second sharp inhale pops the collapsed ones back open, giving the lungs more surface area for gas exchange on that breath. The long exhale that follows offloads a larger share of carbon dioxide than an ordinary breath and stretches the out breath well beyond the in breath. That ratio matters, because the length of the exhale relative to the inhale is one of the more reliable ways to nudge a nervous system toward its parasympathetic, rest and recover side, away from the sympathetic, alert and braced side it tends to default to across a demanding day.[2] None of this needs conscious effort once the pattern is set. You are not thinking your way calm. You are changing the mechanics of a breath in a way the nervous system reads as a signal, which is a different and more reliable thing.

What the Stanford study actually found

The 2023 trial researchers at Stanford ran, led by Balban and colleagues and published in Cell Reports Medicine, is worth reading honestly rather than as a headline. It was a month long, remote, randomised study in a general adult sample, comparing five minutes a day of cyclic sighing, the pattern above, against box breathing, a slower cyclic hyperventilation pattern, and a mindfulness meditation control.[1] Every group improved somewhat, which is a useful reminder that setting aside five quiet minutes a day does something on its own.

Within that, cyclic sighing produced the largest gains, but the size of those gains should be stated plainly rather than oversold: small, statistically meaningful daily improvements in mood, alongside a modest fall in resting respiratory rate over the month, in people who were not selected for high anxiety or a diagnosed condition.[1] It is not a single breath that switches off stress. It is a small, cumulative shift that builds with daily repetition, in the same family of effect as regular exercise or consistent sleep, not a light switch.

That distinction is worth sitting with, because a lot of what circulates online about the physiological sigh strips out the caveats. The study measured mood on a standard scale and respiratory rate, not anxiety disorders, chronic pain or sleep quality directly, and it ran in otherwise healthy adults rather than a clinical population. It is honest evidence for a modest, genuinely useful daily practice, not a substitute for addressing what is actually keeping a nervous system on alert.

How it compares to box breathing

Box breathing, four counts in, four counts held, four counts out, four counts held, was one of the techniques tested in the same trial, and it also improved mood and lowered respiratory rate over the month. Cyclic sighing simply did it a little more, most likely because the emphasis on a long, unforced exhale rather than an equal four part rhythm leans harder on the mechanism above.[1] Box breathing is not a lesser technique. It is a steadier, more structured pattern that some people find easier to hold onto under pressure, and either is a reasonable place to start.

The exhale is where the nervous system is listening.

How to do it

  • Breathe in through your nose until your lungs feel comfortably full.
  • Without exhaling, take a second short, sharp inhale through your nose, on top of the first.
  • Let a long, slow exhale out through your mouth, taking roughly twice as long as the two inhales combined.
  • Repeat for one to three minutes, or longer if it feels useful.

How often

Most people get the steadiest benefit from a short daily practice, one to five minutes, at a time that suits them: first thing in the morning, before bed, or in the gap between finishing work and walking in the front door. It is also worth keeping in reserve for the moment itself, before a difficult conversation, in a car park before a meeting, or lying awake with a racing mind. A single round will not undo a stressful day, but it is a genuinely useful pause button while a longer regulation plan does the rest of the work.

Is it safe for children

Yes, with a parent present and no forced breath holding involved. Children generally take to it more easily than adults when it is framed as play rather than technique: two quick sniffs like smelling a flower, then a long, slow breath out like blowing out a candle from across the room. Keep it gentle, let them stop whenever they want, and do not push for a set number of rounds.
